Over-the-Counter (OTC) Medication Form

This form will allow your child to receive certain medications at school. This will be good for your child’s tenure at John Carroll, unless you notify the nurse’s office of any changes.

Physicians Authorization Form

This form is necessary for any medications not included on the OTC form, either prescription or over the counter. There are two parts to this form, Part 1 must be filled out by the doctor. Part 2 should be filled out by the parent or guardian. This form must accompany the medication in the prescription package. This authorization form is in effect for only the current school year.
